Responsibilities
- Provide high-quality 1:1 nursing care in patients' homes
- Assess patient needs and implement individualized care plans
- Deliver treatments and skilled nursing services within the RN scope of practice
- Monitor patient progress and communicate changes to the care team
- Maintain accurate, timely, and compliant clinical documentation
- Follow all company policies, clinical protocols, and regulatory requirements
- Ensure HIPAA compliance and protect patient confidentiality

- Active, unrestricted Registered Nurse (RN) license in Illinois or a Compact Multi-State RN License valid in Illinois
- Minimum of 1 year of nursing experience
- Drivers License, reliable transportation and auto insurance
- Ability to pass background screening
- Knowledge of state and federal healthcare regulations
- Strong clinical, communication, and critical thinking skills
- 3-5 years of clinical nursing experience
- Previous experience in home health, primary care, or a healthcare facility
- Bachelor's Degree in Nursing (BSN)
- Graduate of an accredited nursing program
- Ability to stand, walk, bend, squat, kneel, reach, push, pull, and lift during patient care activities
- Ability to lift up to 50 pounds with or without reasonable accommodation
- Visual and hearing acuity sufficient to safely perform nursing duties
- PRN / As Needed
- Flexible scheduling based on patient needs and nurse availability
