Job Responsibilities

  • Deliver skilled nursing care in patients' homes in accordance with Florida's Nurse Practice Act, physician orders, agency policies, and accepted standards of professional nursing practice.
  • Perform comprehensive patient assessments, develop, implement, and revise individualized Plans of Care, provide skilled nursing interventions, and monitor patient progress through ongoing reassessments.
  • Document patient assessments, interventions, outcomes, and clinical summaries accurately and in compliance with agency, state, federal, accreditation, and payer requirements.
  • Monitor patient conditions, communicate significant changes to physicians, the Director of Nursing (DON), and the interdisciplinary care team, and collaborate to ensure coordinated, patient-centered care and positive clinical outcomes.
  • Educate patients, families, and caregivers on disease management, medications, treatment plans, and available healthcare resources to promote independence and improved health outcomes.
  • Assist the DON with clinical oversight by supporting staff competency evaluations, supervision, education, mentorship, quality assurance, performance improvement, audits, and case reviews.
  • Maintain compliance with all applicable regulations, agency policies, and professional standards; participate in ongoing professional development; foster positive relationships with patients, families, referral partners, and healthcare providers; and perform other duties as assigned to support exceptional patient care and agency operations.

Education

  • Bachelor of Science in Nursing (BSN) preferred.

Licensure & Experience

  • Current, unrestricted Registered Nurse (RN) license in Florida.
  • Minimum of one (1) year of nursing experience required; home health or community-based nursing experience preferred.

Knowledge, Skills & Abilities

  • Case management experience or knowledge is preferred.
  • Flexibility to work weekends, evenings, nights, holidays, and participate in an on-call rotation, as needed.
  • Working knowledge of home health regulations and compliance requirements, including applicable state and federal regulations, OSHA, HIPAA, and agency policies.
  • Proficient in electronic health records (EHR) and clinical documentation systems, or the ability to quickly learn new technology.

Working Conditions

  • Travel within the assigned service area is required to provide patient care, attend meetings, and complete required training.
  • Provide care in a variety of settings, including private residences, assisted living communities, hospitals, and skilled nursing facilities.
  • Work independently in diverse home and community environments while adapting to changing patient needs.
  • Respond to urgent or emergency patient situations as needed.
  • May be exposed to bloodborne pathogens and other potentially infectious materials (OPIM). Required PPE and infection prevention protocols must be followed.
  • Frequent standing, walking, bending, reaching, lifting, and other physical activities are required to safely provide patient care.
