Responsibilities
- Provide individualized, one-to-one care in the patient’s home environment.
- Implement and manage the established plan of care using sound clinical judgment and critical thinking skills.
- Maintain accurate, thorough, and timely documentation in accordance with the care plan, nursing standards, and regulatory requirements.
- Perform nursing skills, treatments, and procedures within the RN scope of practice as defined by the state Nurse Practice Act, while adhering to company policies, procedures, and clinical protocols.

- Bachelor’s degree in Nursing (preferred)
- Current, unencumbered Registered Nurse (RN) license in Illinois
- Experience caring for patients with tracheostomy, ventilator, and G-tube
- Minimum of one (1) year of documented nursing experience
- Strong nursing assessment and critical thinking skills
- Excellent interpersonal and communication abilities
- Knowledge of regulatory compliance, including state and federal regulations such as OSHA and HIPAA
- May be required to respond to emergency situations
- Tasks may involve exposure to blood and other potentially infectious materials (OPIM), including body fluids and tissues
- Use of Personal Protective Equipment (PPE) may include gloves, masks, eye/face protection, and disposable outerwear or lab coats
- Ability to stand, walk, squat, bend, kneel, reach, twist, push, and pull
- Assistance requirements vary depending on patient needs
- Ability to lift up to 50 pounds
- Adequate visual and hearing acuity for safe patient care
