Job Responsibilities
- Provide skilled nursing care in the home setting
- Perform tasks such as G-tube care, tracheostomy care, oxygen therapy, and suctioning
- Monitor patient condition and report changes
- Follow individualized care plans
- Complete timely and accurate documentation
- Assist with daily routines (including school/day prep when applicable)

Skills & Qualifications
Qualifications
- Active LPN license (Florida)
- Experience with G-tube, trach, or ventilator care preferred
- Strong clinical judgment and communication skills
- Dependable, compassionate, and patient-focused
