What You’ll Do
- Provide clinical leadership and oversight for all patient care services, ensuring quality, safety, and compliance
- Conduct initial assessments, reassessments, and occasional direct patient care (including skilled nursing tasks such as infusions)
- Step in to support the team when needed, ensuring continuity of care
- Supervise, mentor, and develop nursing and caregiving staff through coaching, training, and performance evaluations
- Lead clinical onboarding and ensure competency validation for all new hires
- Oversee care planning, documentation, and clinical audits to maintain the highest standards
- Serve as the primary point of contact during Joint Commission surveys and ensure ongoing survey readiness
- Collaborate with physicians, families, and community partners to coordinate comprehensive care
- Develop and implement policies, procedures, and quality improvement initiatives
- Monitor clinical outcomes, documentation, and compliance with regulatory standards

Skills & Qualifications
EDUCATION

- Bachelor's degree in nursing preferred; Associate’s degree in nursing or equivalent industry experience required.
- Active and unrestricted Registered Nurse (RN) license in state for which care is being provided.
- Basic Life Support (BLS) certification required.
- Minimum of six (6) years of clinical experience required.
- Minimum of two (2) years leading a team of clinicians to include RNs and LPNs experience required. Preferably, in home healthcare.
- Strong understanding of federal and state home health regulations and Nurse Practice Act. Joint Commission experience preferred.
