We are partnering with a world-renowned hospital to deliver an innovative Hospital at Home program that brings acute-level care directly into patients’ homes. This model bridges inpatient hospital care with home health, improving patient comfort and outcomes.
We are currently seeking an experienced and compassionate Registered Nurse (RN) with a background in Med-Surg, PCU, Telemetry, ER, or ICU to provide high-acuity care in a home-based setting.
Shift Options (PRN):
- 06:00 – 14:00
- 15:00 – 23:00
- Flexible 4-hour blocks (example: 08:00 – 12:00)
Responsibilities

- Deliver compassionate, high-quality nursing care in patients’ homes
- Perform comprehensive patient assessments
- Administer medications, treatments, and procedures
- Monitor patient conditions closely and intervene early to prevent deterioration
- Collaborate with physicians, therapists, and the interdisciplinary care team
- Educate patients and families on disease management, medications, and self-care
- Use telemedicine tools and remote patient monitoring technology
Skills & Qualifications
- Active, unrestricted Arizona Registered Nurse (RN) license
- BSN or ADN with relevant experience
- Minimum of 1 year of experience in Med-Surg, PCU, Telemetry, ER, or ICU
- Current Basic Life Support (BLS) certification
