Join our team as we partner with a world-renowned hospital on an exciting and innovative Hospital at Home program. This groundbreaking care model delivers hospital-level services directly to patients in their homes, bridging the gap between acute inpatient care and traditional home health.
We are seeking a compassionate and skilled Registered Nurse (RN) with experience in Med-Surg, PCU, Telemetry, ER, or ICU to provide high-acuity care in the home setting. This role offers the opportunity to be at the forefront of an innovative program that prioritizes patient comfort, continuity of care, and improved outcomes.
Shift: 15:00 – 23:00
Status: Full-Time
Responsibilities

- Deliver compassionate, high-quality nursing care in patients’ homes
- Perform comprehensive patient assessments
- Administer medications, treatments, and procedures
- Closely monitor patient conditions, identifying early signs of complications and intervening proactively
- Collaborate with physicians, therapists, and interdisciplinary care teams to coordinate and optimize care
- Educate patients and families on disease management, medication administration, and self-care
- Utilize telemedicine tools and remote monitoring technology
Skills & Qualifications
- Active, unrestricted Arizona Registered Nurse (RN) license
- BSN or ADN with relevant clinical experience
- Minimum of 1 year of experience in Med-Surg, PCU, Telemetry, ER, or ICU
- Current Basic Life Support (BLS) certification
